Cloze NCLEX questions give you a clinical sentence with blanks in it, and each blank opens a small menu of options. They feel easier than they are, because the format looks like fill-in-the-blank and students answer them the way they read: left to right, one blank at a time.
That habit is the problem. The blanks are rarely equally difficult, and the easiest one is often not the first one. Solving them in reading order means guessing on the hardest blank before the easier blanks have told you what the sentence is about.
There is also a scoring detail worth knowing before you answer. Most drop-down items score each blank independently, so one wrong blank costs you only that blank. One type does not work that way, and on that type a half-correct answer earns nothing at all.

Client data usually appears alongside the item in chart-style tabs. As with other NGN formats, the correct answer often depends on a cue sitting in a tab you have not opened yet.
How Are Cloze and Drop-Down Questions Scored?
This is where the two families separate, and the difference should change how you answer.
Standard drop-down cloze and drop-down table
These use the 0/1 scoring rule. Each drop-down is worth one point and is treated as conceptually identical to a multiple-choice question, just presented differently. Points accumulate as you select the correct option in each blank, an incorrect option earns zero for that blank, and the item total is the sum of the blank scores.
Two things follow from this:
Blanks are independent. Getting the first blank wrong does not invalidate the second. A three-blank item where you get two right is worth two points. This is worth internalising, because students who realise mid-item that their first answer was probably wrong often panic and second-guess answers that were fine.
There is no penalty for a wrong selection. Nothing is subtracted. Since the exam will not advance until every blank is filled, hesitating has no defensive value. Reason it through and commit.
Drop-down rationale
Rationale scoring works differently. Points are earned only when both responses in the pair are correct. NCSBN applies this rule to items assessing paired information, where a concept requires justification through a rationale, along the lines of “the nurse must perform action X because of Y.”
The consequence is direct. On a rationale item, choosing the right action with the wrong reason earns nothing for that pair. Being half right is scored the same as being entirely wrong.
That makes rationale items the one place in this format family where you should slow down and check that the two halves genuinely support each other, rather than checking each half on its own merits.
A note on what 0/1 scoring means
Some study sites describe 0/1 scoring as “all or nothing.” That is a misreading. The 0 and the 1 refer to each individual element, and the item total is the sum of those elements. A five-blank item scored 0/1 can return anything from zero to five. Partial credit is exactly what the rule produces.

Step 1: Read the whole sentence before opening any drop-down
Read the statement start to finish with the blanks treated as gaps. You are working out what the sentence is claiming, not what goes in it yet.
Opening a menu first pulls your attention onto four plausible-looking options before you know what the sentence needs, and those options start doing your thinking for you.

Step 2: Identify what type of answer each blank wants
Now open each menu and look at the kind of options it contains rather than the options themselves. One blank might hold conditions, another lab values, another interventions.
This tells you the shape of the sentence even when the clinical picture is still unclear. A blank offering four conditions is asking you to commit to a hypothesis. A blank offering four interventions is asking what you would do about it.
Step 3: Solve the blank you are most certain about first
This is the step that changes results, and it is the one most students skip.
Work like you would a crossword. Find the blank where the client’s data points most clearly to one option and answer that one first, regardless of where it sits in the sentence. Sometimes that is the last blank.
The reason this works is that the blanks in a well-written item describe one coherent clinical picture. Locking in the blank you are surest about tells you something about the others.
Step 4: Use that answer to narrow the remaining blanks
With one blank fixed, re-read the sentence and ask which of the remaining options are still consistent with it. Usually at least one distractor in each remaining menu was written for a different condition and can now be eliminated on sight.
Work outward from your anchor blank rather than returning to reading order.
Step 5: Read the completed sentence back as one statement
Read your finished sentence through as though you were charting it. Does it describe a client a nurse could actually be looking at?
Students who skip this step submit sentences that are individually defensible blank by blank but describe two different patients. The read-back takes about fifteen seconds and catches that.
One caution. On a standard cloze item, if the read-back makes you doubt an earlier blank, evaluate that blank on its own evidence. The blanks are scored independently, so a doubt about blank one is not a reason to change blank three. On a rationale item the opposite applies, because the pair is scored together.
Worked Example 1: Drop-Down Cloze
The following is an original teaching example written for this article. It is not an actual exam item.
Scenario: A 68-year-old client admitted three days ago for pneumonia has become increasingly confused since yesterday. The client has no history of cognitive impairment. Vital signs are stable and the client is afebrile today. Urine output is 25 mL per hour and the urine appears dark and concentrated. Serum sodium is 118 mEq/L. Serum potassium is 4.1 mEq/L. Blood glucose is 132 mg/dL. The client has been drinking water freely and reports no thirst.

Working through it:
Applying the method, blank 2 is the surest one. Of the four values offered, only the sodium is abnormal. Potassium and glucose are within normal limits and the absence of fever is not a finding that explains confusion. Answer blank 2 first even though it sits in the middle of the sentence.
With a sodium of 118 anchored, blank 1 narrows quickly. Hyperosmolar hyperglycemic state requires a markedly elevated glucose, which this client does not have. Hypovolemic shock does not fit stable vital signs. That leaves the two sodium and water disorders, and the discriminating cues are the low urine output with dark concentrated urine, plus the absence of thirst. Diabetes insipidus produces large volumes of dilute urine and intense thirst. This client shows the opposite pattern, which points to SIADH.
Blank 3 now follows. In SIADH the body retains water, diluting the sodium. Adding more water makes it worse, which rules out encouraging fluids and rules out a hypotonic solution. Insulin has no role here. Fluid restriction is the first action.
Completed sentence: The nurse determines the client is most likely experiencing syndrome of inappropriate antidiuretic hormone based on the client’s serum sodium of 118 mEq/L, and should first restrict oral fluid intake.
The read-back holds together. One client, one problem, one first action.
Scoring: Three blanks, one point each, three points maximum. Because the blanks are scored independently, a student who identified the sodium and the fluid restriction but chose diabetes insipidus in blank 1 would still earn two points. Nothing is subtracted for the incorrect blank.
Worked Example 2: Drop-Down Rationale
Original teaching example.
Scenario: A client with acute kidney injury has a serum potassium of 6.8 mEq/L. The provider prescribes intravenous potassium-lowering therapy. The nurse is preparing to place the client on continuous monitoring.

Working through it:
Severe hyperkalemia affects cardiac conduction, so the risk is dysrhythmia and the monitoring that detects it is continuous cardiac monitoring. Both halves point to the same physiology.
The teaching point is what happens when only one half is right. A student who selects continuous cardiac monitoring but pairs it with respiratory depression has chosen the correct nursing action for this client. Under rationale scoring, that pair earns nothing, because points are awarded only when both responses in the pair are correct.
This is why rationale items reward a different check than cloze items. Do not evaluate the two halves separately and feel satisfied when each seems reasonable on its own. Ask whether the second half is the reason for the first. If your answer reads as two true statements sitting next to each other rather than one statement explaining the other, look again.
Common Mistakes on Cloze and Drop-Down Questions
Answering left to right. The leftmost blank is frequently the hardest, because it often asks for the conclusion the other blanks are evidence for. Answer the evidence first.
Opening the menus before reading the sentence. Four plausible options seen early will shape your reading of the scenario.
Assuming one wrong blank ruins the item. On standard cloze items it does not. Students who lose confidence after blank one often change correct later answers.
Treating a rationale item like a cloze item. Half right earns nothing on a rationale pair. This is the single most costly misunderstanding in this format family.
Not scrolling the menu. Some option lists extend below the visible area. If none of the visible options fit well, check whether there are more.
Choosing an option because it is a true statement. Every distractor is usually true of some client. The question is whether it is true of this client and fits this sentence.
Ignoring the chart tabs. As with every NGN format, the discriminating cue is often in a tab that has not been opened.
How to Practice Cloze Questions Effectively
Two habits are worth building specifically for this format.
First, write the completed sentence out in full when you review. Not the letter of the option you picked, the whole sentence. Cloze items are testing whether you can construct a coherent clinical statement, and reading your own completed sentence back is the fastest way to see whether you actually built one or just filled slots.
Second, track which blank position you get wrong. If your errors cluster in the condition blank, that is a hypothesis problem and points to content review. If they cluster in the action blank while your conditions are right, that is an intervention-priority problem and needs a different fix. If they scatter, the issue is usually process, most often answering in reading order. These three failures look identical on a score report and need completely different responses.
