The glucose trend and the respiratory rate are the straightforward rows. A falling glucose is the expected response to insulin. A respiratory rate dropping from 32 toward normal suggests the Kussmaul respirations are resolving as the acidosis corrects.
The potassium row is the one that catches people. A potassium of 3.2 looks like it is moving toward normal from a high starting point, and students classify it as improvement on that basis. Insulin drives potassium into the cells, so a rapid drop during DKA treatment is a recognised complication, not a sign the treatment is working. The direction of the number is not the point. The cause is.
The neurologic row is the second trap. A headache with decreasing arousal during DKA correction raises concern for cerebral edema. Read on its own, a headache sounds minor. Read in context, it is the most urgent row in the table.
The IV site row demonstrates something worth noticing: not every row is about the condition. Some rows exist to test whether you can recognise information that is real but irrelevant to the judgment you were asked to make.

Work down one column at a time. Start with hepatic encephalopathy. Lactulose reduces ammonia absorption, so it belongs. Asterixis is a hallmark neurologic finding, so assessing for it belongs. Level of consciousness is central to grading encephalopathy, so it belongs.
Then move to variceal bleeding. Large-bore access, serial hemoglobin and hematocrit, and preparation for band ligation all follow from an acute bleeding risk.
The row that decides this item is level of consciousness. It belongs in both columns. A client who is bleeding from varices can develop altered consciousness from hypovolemia, from blood in the gastrointestinal tract raising ammonia, or from both. Students who assume each row belongs to exactly one column will select it once and lose the second point.
The instruction line told you interventions may apply to more than one complication. That sentence exists for a reason. Read it.
Scoring: This item uses plus/minus scoring. Each correct cell adds a point and each incorrect cell subtracts one, with the totals summed by column and any negative column rounded up to zero.
So if you selected all three correct cells in the encephalopathy column plus one wrong cell, that column nets two. If you then made three wrong selections in the bleeding column against one correct one, that column would be negative and is rounded to zero instead. Your item score would be two, not less. The floor protects you across columns. It does not protect you within one.
Common Mistakes on Matrix Questions
Answering before checking the type. The correct strategy on one type is the wrong strategy on the other. Two seconds spent reading the instruction line is the highest-value time you will spend on the item.
Over-selecting on multiple response items. Adding a cell you cannot justify is a coin flip with a penalty attached. If you cannot name the cue that supports it, leave it.
Under-selecting because a row seems to belong to one category. The reverse error. On multiple response items, check whether the instructions allow a row to appear in more than one column, then check each row against every column.
Assuming columns must be evenly filled. There is no rule requiring a balanced distribution. Some columns get one row. Force-fitting rows to even out the grid is a reliable way to convert a correct answer into a wrong one.
Judging a finding by direction instead of by cause. A number moving toward normal is not automatically good, as the potassium row in Example 1 shows. Ask what caused the change.
Drifting between rows. By row six, students often apply a slightly different standard than they did at row one. The pattern check catches this.
